Ladies, it has been a very busy summer for us at National
Headquarters and for many of you, however, I hope you have had a
few moments of "rest and relaxation."
I hope the following information will be helpful to you as you
begin to function again.
I. Copies of Series 100 Forms
A packet of updated and revised forms to help facilitate a
smooth running administration is enclosed. The exception is
Form 105 - Transfer Form, which is requested by the member
and Form 106 - The Master Data Form, which is sent out in
January by National Headquarters to each chapter.
II. 1986-87 President and Program Director List
Many chapters have reported their officers for 86-87. If you
have not reported this information, please do so immediately.
It is imperative that we have written confirmation of the
chapter president and her current address (see the enclosed
form). Please be sure we have this information. Chapter
presidents are our link to the chapter membership. Please
make a copy of Form 107 to report any membership changes
throughout the year.
III. Form 990
A. Be sure to fill these forms out correctly (see enclosed
sample) and submit it to the IRS by September 15th. When
answering letter E, all chapters should have the same
information in this space. The four digit group
exemption number is 5750. Do not confuse the group
identification number with your own 990 number that the
IRS assigned your chapter. New chapters who have not
applied for 990 numbers should do so immediately. The
form may be picked up at your local IRS office.
Each chapter should therefore have two numbers: the
chapter's 990 number (which is a seven digit number) and
