INSTRUCTIONS
1 Indicate county of residence.
2. Indicate year model of vehicle, trailer, machinery, or motorcycle.
3 Indicate make of vehicle or trade name.
* If trailer is shopmade or homemade, indicate that as make.
4 Indicate body style.
* For commercial vehicles - pickup, dump, garbage, box van, utility, flatbed, or truck tractor.
* For trailers - utility, flatbed, stock, box, livestock, or stake.
5. Indicate vehicle identification number or serial number.
6. For trailers only. Must show empty and gross weight.
* The gross weight is the empty or shipping weight plus the heaviest load to be carried.
Indicate the name of the exempt agency.
8. Indicate the mailing address and zip code for delivery of exempt plates.
* Note: exempt plates will be mailed to this address.
9 Indicate name of person to contact if we have any questions.
10 Indicate daytime telephone number.
1. If your agency currently has exempt plates for unmarked units, please check this box if switching
to standard exempt plates for marked units.
12. Indicate if plates or stickers are lost, stolen, or mutilated.

Texas Department of Transportation.

agency, file this form, the Application for Title, and all evidence of ownership with the local County Tax
Assessor-Collector's office.

Volunteer fire departments are required to submit Form 62-F to obtain exempt license plates.
Emergency ambulance services are required to submit Form VTR-62-EMS to obtain exempt license plates.
Trailers weighing in excess of 4,000 Ibs empty must be titled.
Semitrailers weighing in excess of 4,000 lbs gross must be titled.
A FEDERAL TAX EXEMPTION STATUS OR THE ASSIGNMENT OF A COMPTROLLER I.D. NUMBER

REPLACEMENTS FOR STANDARD EXEMPT LICENSE PLATES
If Standard Exempt License Plates become lost, stolen, or mutilated, this properly executed form must be submitted
directly to the Special Plates Branch in Austin. Immediately upon receipt of the replacement license plates, any plate
remaining on the applicant's vehicle should be removed and surrendered with the exempt license receipt to the Exempt
Section at the above address for cancellation.
For Replacements for ALIAS EXEMPT LICENSE PLATES, use Form VTR-62-G.
For Replacements for REGULAR EXEMPT LICENSE PLATES, use Form VTR-119.
