NOTICE OF APPLICATION
FOR OIL AND GAS WASTE DIS-
POSAL WELL PERMIT
Three P Operating Company, P.O.
Box 490, Newcastle, Texas 76372, is
applying to the Railroad Commission of
Texas for a permit to dispose of pro-
duced salt water or other oil and gas
waste by well injection into a porous
formation not productive of oil or gas.
The applicant proposes to inject fluid
into the Gunsight Sand, Larimore Unit
(RRC#16920), Well Number D1S. The
proposed injection well is located in
T.E.&L. Co. Survey #2246, Abst.
#1079; three miles North of Newcastle
in the Young County Regular Field, in
Young County. The waste water will^e
injected into strata in the subsurface
depth interval from 914' to 938' feet.
LEGAL AUTHORITY: Chapter 27
of the Texas Water Code, as amended,
Title 3 of the Natural Resources Code,
as amended, and the Statewide Rules of
the Oil and Gas Division of the Railroad
Commission of Texas.

persons who can show they are ad-
versely affected-, or requests for further
information concerning any aspect of
the application should be submitted in
writing, within fifteen days of publica-
tion, to the Underground Injection

PUBLIC NOTICE OF MEDI-
CARE TERMINATION OF
PROVIDER AGREEMENT
Hamilton Hospital of Olney

On October 28, 1994 the Medicare
agreement between Hamilton Hospital
of Olney and the Secretary of Health and
Human Services relating to the
hospital’s participation in the Health
Insurance for the Aged and Disabled
Program (Medicare) is to be terminated,
unless the hospital is able to achieve
compliance with the Medicare require-
ments prior to that date. The cause for
Medicare termination is that Hamilton
Hospital of Olney does not meet mini-
mum health and safety standards re-
quired for Medicare program participa-
tion. The hospital is not in compliance
with the Medicare Condition of Partici-
pation for Quality Assurance.
The Medicare program will not make
payments for hospital services fur-
nished to patients admitted to Hamilton
Hospital of Olney on or after October
28,1994. For patients admitted prior to
October 28, 1994, payment may con-
tinue for up to 30 days of covered serv-
ices furnished on and after October 28,
The Olney Cemetery Board of the
City of Olney will receive sealed bids on
a Cemetery Maintenance Contract for
Restland and Pioneer Cemeteries at
Olney, Texas.
The requirements for the one year
contract are: mow, trim, water and care
for trees and shrubs, fill graves when
soil has settled, remove old floral
wreaths, debris and rubbish, work for
good grooming of both cemeteries, and
alert Board Chairman to problems and
areas that need attention. Bidder must be
available to contribute sufficient time to
carry out these duties. No sub-contract-
ing allowed.
Bidder is required to furnish all mate-
rials, tools and equipment necessary to
perform the foregoing services. Bidder
assumes all liability for property dam-
age and to his machinery, tools and
injury to his employees in performance
of the duties herein set out.
Bids will be received until 2 P.M.
November 7, 1994 at the Olney City
Hall, 201 E. Main St.
