birthday, a booster is re-
quired.
3. A booster dose is required
every 10 years.
B. Polio
1. Requirements — 3 doses.
2. If these doses have been re-
ceived before the fourth
birthday, a booster is re-
quired.
C. Measles — History of illness
or one dose to all children
through age 11 years.
D. Rubella — One dose to all
children through age 11
years.
E. Tuberculin test — All first
and seventh grade students,
and out-of-state students.
Must be given not more than
6 months prior to entering
school. Kindergarten in-
cluded.
