attached, and total amount due sheriff for fees and
mileage;
b. Itemization of fees, showing name and post office
address of each witness, date of service, amount due for
serving attachment, number of miles traveled to attach
witness, mileage due, total amount due for service to
each witness, total amount due, amount due for approving
bonds, and grand total due sheriff;
c. Approval of district clerk, showing date witnessed
and signature of clerk or deputy;
d. Certification by district clerk that named witness
disobeyed subpoena before issuance of attachment;
