law before subpoena issued, showing name of county,
name of clerk, and signature of deputy;
d. Approval of claim by presiding judge, showing
name of judge, judicial district number, name of witness,
amount due witness, and signature of presiding judge;
e. Attestation of clerk, showing name of witness,
amount due witness, and signature of clerk or deputy.
Also includes certification of recordation, showing
county, name of witness, book and page number where recorded,
and signature of clerk or deputy. Arranged
