Balderas has the ability to run over many opponents and he has shown his
ability to outrun the opposing team's defenders many times.
Many arc concerned about grow-
ing old and the loss of thinking func-
tions or know of someone who is
affected. This is usually a gradual
process that is slow and irreversible.
The technical term is dementia
which is literally a loss of mind.
More precisely, this is a deterioration
of all cognitive and intellectual func-
tions without disturbance of con-
sciousness, awareness, or percep-
tion. Only about one out of twenty is
affected for those oversixty,butupto
one out of five may be affected for
those over eighty.
A large number of disease proc-
esses may cause dementia, and some
of these are treatable. Although
dementia may be the only evidence
of a disease process, many have cer-
tain findings on morcdctailcdcxami-
nalion of the nervous system and
remainder of the other organ sys-
tems. Certain dementias also have
typical laboratory findings. Because
about 15% of the dementias arc treat-
able, the physician typically docs a
routine work-up to try to establish the
cause.
The first step in the work-up is a
thorough physical exam and history.
Because the memory is affected, one
of the caregivers who knows the
individual well is interviewed in
addition to the patient. Routine lab
tests include a complete blood count
and scrum chemistries. Vitamin and
thyroid levels are checked. A special
type of X-ray of the brain is done
called CT scan. This gives cross
sectional views of the brain and can
often show if small strokes have
occurcd or the presence of increased
pressure in the brain. On certain
occasions other tests will be done
such as an EKG (electrical activity of
the heart), EEG (electrical activity of
the brain), chest X-ray, or spinal lap.
Memory is a complex process that
is still poorly understood. It involves
several steps from the actual experi-
ence to recall and reproduction of the
memory. There arc additionally
scrveral types of memory. Immedi-
ate recall lasts seconds, short-term
memory lasts minutes to hours, and
long-term for years. Each of these
arc tested during the exam.
The most common cause of de-
mentia is Alzheimer's disease. The
only way to be absolutely sure of the
diagnosis is a biopsy of the brain at
autopsy. Clinically it is a diagnosis
of exclusion; other causes that arc
similar must be ruled out. There arc
now some experimental blood tests,
but their validity is not yet known and
they aren't widely available. The
disease typically lasts between six
and ten years though great variation
exists. Alzheimer's disease pro-
gresses through three stages.
Initially there arc mild memory
changes and cognitive impairment.

Forgetfulness, lack of initiative, dis-
tractabilily, reduced comprehension,
and personality changes may be
noted. The neurological exam by a
physician is usually normal at this
point.
The second stage is similar, but
the changes arc greater. Complex
tasks may be impossible, Difficulty
finding words may be prominent,
concentration is poor, and those af-
fected are unaware of why they have
decreased abilities. Apathy and
mood swings occur frequently, and
paranoid ideas and dclusiions may be
present. Remote and recent memory
is impared, and there may be an in-
stinctive wilhdrawl from stressful
situations. The final stage involves
severe impairment: fecal and urinary
incontinence arc common, and ver-
bal output is decreased, occasionally
totally to mutism. Confinement to
bed is required.
Treatment involves symptomatic
relief for the patient and support for
the family. An excellent book that
gives detailed explanations for most
common questions and care for those
with dementia is THE THIRTY-SIX
HOUR DAY by Nancy Mace and
Peter Rabins. It is easy reading and is
a must for families with one affected
by dementia. Support groups are also
available such as THE

notch in the Harris poll to number
19 this week. The Owls arc unde-
feated and have a power rating of
165. The Owls arc picked by 33
points over a Wall Hawk team that
traditionally plays RCHS much
tougher than the picks.
Other undefeated teams in class
2-A through this week include:
Boling, Cooper, DeLeon, Electra,
Farmcrsvillc, Idalou, Highes
Springs, Manor, Overton, Mason,
LaPryor, and Refugio.
