'Horne health care offers nurses more opportunities than
the traditional hospital setting • independence and autonomy
while budding personal relationships with patients, one on one,*
said Bob Fuaoo, praaident of Olsten Kimberly Quality Cara, the
largest home health care provider in North America. In order to
support the shift from hospital to home, Fusco says that his
company offers nursing ongoing education and training for
making the career switch.
Reflective of this shift, the Bureau of Labor Statistics re-
ports that the relative numbers of Jobs in home care is increas-
ing at five times the rata of hospital employment. And, by the
year 2005, more than 500 thousand new Jobs will be created In
the home care industry, currently the second fastest growing in-
dustry in the economy, according to the bureau.
Why the switch to home care? New technology now al-
lows nurses to perform sophisticated medical producers in the
comfort and convenience of home. Once only possible in a
hospital, today, high-tech treatment such as cancer chemother-
apy, neonatal care and cardiorespiratory monitoring are com-
mon in this patient-focused environment.
This technology allows for patients to receive personal-
ized care in the comfort and convenience of home, for a fraction
of the cost of hospital care. That’s why many employers and in-
surers are increasing depending on home care to meet their
health care requirements, and why, according to a Louis Harris
1992 survey, Americans support home care 9 to 1 over other
forms of institutional care.

In light of this trend, the demand for more registered
nurses for home health care is likely to continue, making regis-
tered nursing a highly attractive profession.

Is Growing Strong
Americans are expected to
spend more than $1 trillion dol-
lars for personal health care in
1995.
Of that amount, only 3 per-
cent will go to home health
care, one of the least expensive
, yet fastest growing areas in
| America's health care system.
The National Association
( for Home Care (NAHC) has
•?nn|i Identified a total of 17,561
home care agencies providing
services to some 7 million indi-
viduals who need them because
of acute illness, long-term
health conditions, permanent
disability or terminal illness.
In Texas, home care is
available on two levels - fed-
eral and state. At the federal
level, Medicare and Medicaid
pay for a variety of home care,
including skilled nursing, for the
medically disabled, elderly and
low incomed. At the state level,
Community Care Services pro-
grams pay for nonskilled atten-
dants to assist those who are fi-
nancially and medically eligible
with their daily living tasks.
While doctors must pre-
scribe skilled care, many of
those receiving attendant serv-
ices from Community Care are
referred to home care agencies
by case workers with the Texas
Department of Human Services.
The first home care agen-
cies were established in the
U.S. in the 1880's. That’s when
public health nurses traveled to
patients' homes caring for the
sick, teaching family members
how to provide care and sug-
gesting ways to improve health
and comforting the dying.
As efforts continue to lower
the cost of health care in
America, home care is ex-
pected to continue its strong
growth.

Because hospital stays are considerably shorter, or

time skilled nursing care, physical therapy or
speech therapy.
(2) The patient is confined at home or has great
difficulty leaving home, even if home is in a sen-
ior living facility. The patient does not, by any
means, have to be bedridden to receive home
health care.
(3) A doctor determines the need for home health
care and plans treatment for the patient.
(4) The home health agency providing services is a
Medicare Participant.
