Dr. Leonard Bentch announces that he will close his office for the practice of medicine
effective December 31, 2005. After nearly 30 years of medical practice including 8 years
in Fredericksburg, he hopes to spend more time with his family and grandchildren.
Unfortunately, there is not anther gastroenterologist to take over Dr. Bentch’s practice. His
patients can ask their primary care physician to refer them to another gastroenterologist.
Upon each visit, both for initial consultation and follow-up office visit, Dr. Bentch has
sent to the referring physician a copy of the patient’s medical record. Copies of all endo-
scopic procedure (gastroscopy and colonoscopy) reports have also been sent to the refer-
ring physician. All laboratory reports, x-ray studies and hospital procedures performed
in Fredericksburg are maintained by and are available from Hill Country Memorial
Hospital. Any laboratory tests or radiographic studies performed outside of Fredericksburg
will be available from that facility. Therefore, both the patient and his or her primary care
physician already have available all the information needed to continue care.
If they so desire, patients may request a copy of their medical records, or arrange for the
transfer of their medical records to another physician, by submitting a signed authoriza-
tion for the release of personal healthcare information to the office of Dr. Bentch. There
will be a charge for copying records.
This notice is posted in accordance with Rule 165.5 of the Texas State Board of Medical
Examiners.
